The content on this website is general information only. It is not medical advice, it is not a diagnosis, and it is not a substitute for assessment by a qualified healthcare provider.

Using this website does not create a therapist-client relationship. That relationship begins only when you book an appointment, complete the intake process, and are seen.

Do not delay seeking care because of something you have read here. If you have a medical emergency, call 911 or go to your nearest emergency department.

Booking

Appointments are booked through a third-party system. Its own terms apply once you leave this website.

Booking an appointment is a request. It is confirmed when you receive confirmation.

Cancellation. You may reschedule or cancel free of charge up to 24 hours before your appointment. A late cancellation, with less than 24 hours’ notice, or a missed appointment is charged a $75 fee.

Payment

This is a cash-pay practice. Insurance is not billed.

Payment is due at the time of service. Current rates are published on this website and may change. The rate that applies is the one in effect when your appointment is confirmed.

Uninsured and self-pay clients have the right to a Good Faith Estimate of expected charges. See the Good Faith Estimate page.

Intellectual property

Everything on this website belongs to Eleve Physical Therapy LLC — text, images, layout, logo, and the assessment instruments and related materials described here — unless stated otherwise.

You may view and print pages for your own personal use.

You may not reproduce, distribute, modify, publish or create derivative works from any of it without written permission, or use any of it for commercial purposes.

Assessment instruments, manuals, scoring criteria and related materials are licensed separately and are not covered by this permission. Nothing on this website grants any right to use them.

This website links to services and resources operated by others. Those links are provided for convenience. Their content is not controlled or endorsed here, and their own terms and privacy policies apply.

Limitation of liability
