Eleve Physical Therapy LLC Last updated: 1 October 2026
This policy explains what information is collected through this website, how it is used, and the choices available to you.
What this policy covers, and what it does not
This policy covers the website. Inquiries, form submissions, appointment requests, and the technical information collected automatically when you visit.
It does not cover your clinical records. Information created in the course of providing physical therapy — your assessment, treatment records, clinical correspondence — is held confidentially and kept entirely separately from anything collected through this website.
Who we are
Eleve Physical Therapy LLC, a Kentucky limited liability company.
Questions about this policy, or requests relating to your information, may be sent to ellen@eleve-pt.com.

About Jane. Jane App is operated by Jane Software Inc., a Canadian company based in British Columbia. This practice’s Jane account is hosted in Jane’s United States data region, so records held in Jane are stored in the United States. Jane describes itself as a service provider to the practice: it accesses patient information only on the practice’s instructions and does not use it for its own purposes. Jane uses sub-processors for particular features, such as secure messaging, telehealth and transcription, and publishes the list of them.
Legal requirements. Information may be disclosed where required by law, court order, or a professional or regulatory obligation.
Business transfer. If this practice is sold or transferred, information may transfer with it. You would be notified before that happened.
How long information is kept

Interest list entries are kept until you ask to be removed, or until the product they relate to is no longer offered.
Your choices

Ask for it to be deleted. Email and ask. Some information must be retained where law or professional obligation requires it, and you will be told if that applies.
Come off an interest list. Every email sent includes a way to unsubscribe, and you can also ask directly.
Security
No method of transmission or storage is completely secure. Email in particular is not secure and should not be used for health information.
