2026 RATES
You shouldn’t have to email someone to find out what an appointment costs. Elevé is cash-pay and out-of-network, which means no surprise billing and no benefit ceiling on your care.
15-minute discovery call — Free
Talk it through, decide if we’re a fit. No intake forms, no commitment.
Initial evaluation — $150
60 minutes. Full history, assessment, and your starting plan.
Follow-up treatment — $100
45 minutes, one-on-one.
Pre-pointe evaluation — $85
45 minutes. Readiness screening for dancers going en pointe.
Payment
Taken when you book, through Jane. Card, HSA, and FSA all work.
Superbills
I provide a superbill after every visit that you can submit to your insurer for out-of-network reimbursement. Whether they reimburse, and how much, is between you and your plan.
Medicare
I’m not able to see patients who have Medicare as a primary or secondary insurance. Federal rules don’t permit physical therapists to treat Medicare beneficiaries on a cash-pay basis. Get in touch and I’ll point you toward someone who can help.
CANCELLATIONS & RESCHEDULING, IN FULL
Payment is taken at the time you book. You can reschedule at no cost any time up to 24 hours before your appointment — do it yourself in Jane, or message me.
There is a late cancellation fee and no show fee of $75 that will be charged to the card on file.
Illness, injury, an emergency, a show that ran long — these happen, and they’re not what this policy is for. Reach out before your appointment and we’ll sort it out.
Discovery calls are free and carry no cancellation fee — though a heads-up is always appreciated.
HSA and FSA
Physical therapy is an eligible expense. Most clients pay with an HSA or FSA card.

Education and partnership work is quoted separately, based on format, length, and group size.
Not sure how many visits you need?
Neither am I, until we talk. Most people need fewer than they expect.
